Dunia Autis Remaja

Penyandang autisme yang beranjak remaja perlu penanganan lebih khusus. Sebab, mereka punya pemahaman berbeda dan terbatas tentang perubahan fisik dan psikis masa pubertas. Dalam bulan peduli autis April ini, diserukan agar memberikan perhatian ekstra pada pendidikan penyandang autisme remaja.


DYAH Puspita masih menyimpan pesan pendek itu di telepon selulernya. Pesan yang ia terima dua pekan lalu itu dari Ikhsan Pratama, putra tunggalnya, 18 tahun. ”Ibu Ita, Ikhsan mau cari cinta,” begitu tulisnya.
Bagi Ita, ini bukan pesan biasa, karena pengirimnya autis. Seperti remaja sebayanya, Ikhsan rupanya mulai tertarik kepada lawan jenis. Maka Dyah pun membalas: ”Boleh, tapi tidak paksa.” 

Mengenal konsep ”cinta” adalah suatu lompatan besar bagi penyandang autisme seperti Ikhsan. Ia pertama kali mengenal kata itu dari lagu band kesukaannya, The Changcuters. Setelah bolak-balik bertanya kepada sang ibu, Ikhsan pun membentuk definisinya tentang cinta, yaitu ”cewek, cantik, pintar, tidak berisik”.
Belakangan ini, kesadaran dan perhatian masyarakat Indonesia akan autis memang terus membaik. Berbagai kampanye pun diluncurkan. Termasuk lewat causes—semacam petisi—di Facebook bertema ”Stop Using the Word Autis in Daily Jokes” yang diluncurkan pada bulan peduli autis internasional, April ini. Namun penanganan penyandang autisme yang menginjak masa remaja masih menjadi persoalan, bahkan juga di negara maju, yang sudah memiliki tingkat pemahaman dan penanganan autis lebih baik. 

Di masa lalu, autisme dianggap sebagai gangguan psikologis akibat pola pengasuhan yang tidak tepat. Namun penelitian lanjutan membuktikan autisme disebabkan oleh ketidaknormalan pada otak. Barulah pada 1970-an autisme bisa didefinisikan sebagai gangguan perkembangan. Ada tiga ciri utamanya: gangguan berinteraksi sosial, gangguan berkomunikasi, serta keterbatasan minat dan kemampuan imajinasi. 

Di Indonesia, tidak ada data eksak tentang jumlah penyandang autisme. Itu juga karena masih ada orang tua yang tak mau mengakui anaknya autis—karena dianggap aib. Dyah Puspita, yang juga sekretaris Yayasan Autisme Indonesia, menyatakan yang terpenting adalah apa yang bisa dilakukan bagi anak autis, sejak masa kanak-kanak hingga masa remaja. 

Autis atau tidak, masa remaja memang ”membingungkan” bagi banyak orang karena merupakan transisi menuju kedewasaan. Pada periode ini, hormon-hormon mereka berkembang, organ reproduksi sudah berfungsi, penampilan fisik pun berubah. Para pria sudah mengalami mimpi basah dan suara mereka pun berubah. Para wanita mulai ditumbuhi payudara dan sudah mendapat haid. 

Perubahan-perubahan ini juga berpengaruh pada emosi seseorang. Remaja autis pun mengalaminya. Menurut dokter Tjin Wiguna, spesialis kesehatan jiwa dari Rumah Sakit Cipto Mangunkusumo, Jakarta, masa pubertas adalah fase yang kritis dalam perkembangan jiwa setiap orang. Tak aneh, jika tak mendapat arahan yang tepat, banyak remaja yang mengalami krisis identitas. Pelariannya bisa sesuatu yang negatif. 

Karena itu, mesti ada figur yang mampu membimbing si remaja agar memperoleh pemahaman tepat tentang perubahan yang dialami, baik fisik maupun psikis. Urusan perkembangan seksual dan jatuh cinta pun termasuk di dalamnya. Dalam kasus Ikhsan, figur itu adalah Ita, ibu yang mengarahkannya di masa puber. 

Menurut Ita—yang juga psikolog dan pendiri sekolah khusus autis Mandiga—orang kadang lupa memperlakukan anak autis seperti orang normal. Setiap kali akan mengajarkan sesuatu, banyak yang sudah telanjur skeptis: memangnya bisa, memang mengerti? 

Padahal, dengan pendekatan yang tepat, anak dan remaja autis bisa memahami penjelasan tentang sesuatu. Tentunya tidak langsung mencekokinya dengan konsep-konsep yang ”tinggi”, tapi yang dekat dengan kesehariannya. Misalnya, remaja lelaki autis juga mulai tertarik pada bentuk fisik lawan jenis; ada yang berusaha memegang payudara wanita. Jika tidak diarahkan, kata Ita, remaja autis tak paham bahwa menyentuh dada perempuan dewasa itu tidak baik. 

Ketika Ikhsan mulai mengenal konsep cinta, Ita mengarahkan dengan jelas dan instruktif apa yang boleh dan tidak. Misalnya ”Ikshan boleh cinta, tidak paksa” atau ”boleh cinta, tidak pegang karena orang tidak suka dipegang”. 

Berdasarkan pengalaman merawat Ikhsan dan menggeluti bidang psikologi anak, Ita membagi ilmunya untuk sesama orang tua anak autis. Pertama, jangan memberikan arahan abstrak karena akan membingungkan. Misalnya, jika mengajari anak berlaku sopan saat bertemu dengan orang lain, jangan kita mengatakan, ”Kamu harus sopan,” karena anak autis sulit memahami ”sopan”. Langsung saja katakan apa yang harus dia lakukan: ”Salam tangannya.” Kedua, hindari melarang tanpa memberikan solusi. Misalnya, jika kita mau bilang ”jangan lari”, harus ditambahkan dengan ”duduk saja di sini”. 

Tanpa arahan dan petunjuk yang jelas—apalagi melulu hanya larangan—anak autis bisa ”tersesat” dan kebingungan akan konsep dirinya. Ita menambahkan, seperti juga anak non-autis, kebingungan akan konsep diri akan membuat anak tumbuh dengan depresi dan tidak percaya diri. Padahal, khususnya bagi anak autis, memompa kepercayaan diri adalah salah satu hal terpenting. 

Jika tidak diarahkan, kata Tjin Wiguna, remaja normal saja bisa tersesat. Ujung-ujungnya, mereka malah melakukan hal-hal yang negatif, bahkan destruktif, sebagai upaya pencarian identitas. Apalagi bagi mereka yang autis. Tjin pernah mendapati seorang remaja autis yang bermasturbasi di muka umum. Anak itu merasa nyaman-nyaman saja, tak sedikit pun merasa malu atau aneh melakukan onani di depan publik. 

Yang sulit bagi anak-anak autis, perkembangan mental dan emosi mereka tertinggal, tapi pertumbuhan fisiknya sama dengan rekan sebayanya yang non-autis. Mereka yang normal bisa mudah mengobrol, mencari informasi, dan mendiskusikan perubahan-perubahan tubuh mereka. Orang tua dan guru biasanya sudah mengajarkan sebelum tanda-tanda kedewasaan itu datang. 

Tapi tidak demikian dengan anak autis. Jika perubahan yang mereka alami tak mendapat penjelasan memadai dan tepat—lantaran si autis dianggap ”tak mengerti”—ia semakin tenggelam dalam kebingungan dan perasaan tertekan. Di sinilah pentingnya peran orang tua, guru, dan orang-orang terdekat untuk memperlakukan anak sebagai ”remaja biasa” yang butuh informasi serupa dengan cara khusus. 

Ita membeberkan tahap perkembangan lelaki dengan menyodorkan gambar ayah Ikhsan semasa bayi, kanak-kanak, hingga dewasa. Sang ibu menjelaskan bahwa orang yang sama akan berubah sesuai dengan umurnya. Visualisasi gambar memudahkan sang anak memahami konsep yang dimaksud. 

Dokter Tjin memberikan sejumlah saran untuk membimbing para penyandang autisme di usia remaja. Pertama, perkenalkan mereka terus-menerus kepada organ-organ seksual, misalnya payudara atau penis, dengan memakai boneka. Kedua, tuntun mereka tahap demi tahap menghadapi perubahan fisik itu. Misalnya, anak perempuan yang sudah mendapat menstruasi diajari dengan mencontohkan cara memakai pembalut, membuangnya, dan seterusnya. Ketiga, menjelaskan segala sesuatunya dengan visualisasi atau gambar, misalnya, pada mimpi basah yang terjadi. 

Cara-cara ini bisa disesuaikan dengan tingkat intelektualitas setiap anak. Psikiater yang juga berpraktek di Rumah Sakit Pantai Indah Kapuk itu menjelaskan tingkat kecerdasan anak-anak autis sangat bervariasi, dari yang sangat pandai hingga yang memiliki keterbelakangan mental. Pendekatan yang dilakukan mesti disesuaikan dengan kondisi si remaja. 

Untuk mengembangkan kecerdasannya, Ikhsan sejak dua tahun lalu menjalani homeschooling. Seorang guru datang ke rumah tiga kali sepekan. Secara bertahap, remaja penyuka buku dan majalah otomotif ini bisa mengikuti pelajaran matematika, fisika, dan biologi, atau apa saja yang disukainya. Ita tak memaksakan anaknya mengikuti kurikulum sekolah umum. Jadi, buku-buku pegangan Ikhsan ada yang untuk level SMP, juga SMA. 

Salah satu ”hobi” Ikhsan saat belajar adalah menekan-nekan tuts di telepon Nokia Communicator-nya. ”Setiap ada sesuatu yang melintas di kepalanya, dia langsung mengirim SMS ke saya,” kata sang mama yang dia panggil Ibu Ita itu. Termasuk pesan pendek soal cinta tadi.
